A little bush hospital in Banyo, Cameroon with 33 beds has a satellite Internet connection that is available at the ex-pat doctor's house.  His wife graciously lets people barge in and sit at their computer so they can check email pretty much all day.

This is the situation I ran into years ago.  I setup SIAS there and the change was significant.

In the first 3 months, the cost of the Internet connection to the hospital dropped 95% because of users buying credit and using the system.  Instead of paying the full bill each and every month, the hospital only had to pay 5% of it.  Internet access was expanded from just one house to the entire complex.  This included the entire hospital, the radio station, the staff housing, the administrator's house, visiting doctor's house, the school and many houses.  Over 25 users now had Internet access including any hospital visitors.  The hospital now has the ability to offer Internet access to almost every house in the city.
